What to include in your protocol submission packet

1. Signed copy of the protocol [containing signatures of all investigators, supervisor (if PI is student), and department chair]

2. Three copies of the informed consent, flyers, or advertisements.

3. Supplemental items that will be used in the study (questionnaires, interview questions, measures, flyers, advertisements)

4. You will need to submit 3 copies of the protocol packet (one original plus two copies)

The review process usually takes 7 to 10 business days. You will receive an email notification about revisions needed to the protocol. If your study is approved, then the approval packet will be mailed to you at the address you indicated on the protocol submission form.
