What to include in your protocol submission packet

1. Three copies of the signed protocol [containing signatures of all investigators, supervisor (if PI is student), and department chair]
2. Three copies of the informed consent, flyers, or advertisements, interview questions, surveys)
3. If the protocol is funded by NIH we need one copy of the grant proposal.

The review process usually takes 7 to 10 business days.  You will receive an email notification about revisions needed to the protocol. If your study is approved, then the approval packet will be mailed to you at the address you indicated on the protocol submission form.
